What does BenefitWebsite ask me?

We will often have to know your date of birth, home phone number, and address just to get your rates for you. After you have applied for insurance we can see the information on your application for insurance.

Other than name adress and telephone numbers, we do not store any personal information in any database, we use the insurance company database, which they maintain and secure, and we use Security Enhanced Linux software on our computers (the same software used by the National Security Administration) to access the database when you need help with a claim and we have to see the information.

We may disclose your information to insurance companies, or physicians or other medical providers when you require claim service or other insurance services. We only disclose the information that is required to provide the service you request, and which are permitted by law.
